Updating your details

It is important that we always have your up to date contact details.  If you are referred by the GP and your home address details are incorrect, your appointment details will be sent to the wrong address which will result in a delay before you are seen.  Sometimes our receptionists may have to contact you regarding an appointment or reviews.  It is therefore important to always keep us updated with your present telephone number for these reasons.

It is preferable for you to update any of your contact details by completing a short form in the surgery, but we will accept a change of telephone number over the telephone.

If patients do not update the surgery with a change of address, we may presume that you have moved out of the area, in which case the Health Authority may remove you from our GPs list which will result in you not being registered with a doctor.  If you move outside the practice area you may have to register with another practice in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.

 

If you have changed your address or telephone number, please come and see our receptionists. Alternatively  you can download the ‘Patient change of address’ form below, fill it in and bring it along with you at your convenience. (Please provide proof of your new address)
